• If you have a Library Card:
  User Name = Please enter the user name supplied to you. This is usually your last name.
  Number = Please enter your library card number which was supplied to you by the State Library

• If you do not have a library card and are an Arizona state employee:
  User Name = Please enter STATE GOVERNMENT
  Number = Please enter your 10 digit work phone number, starting with the area code.
  
• To apply for a library card, as a state employee, please complete the online application for a library card.
• If you are a Braille and Talking Book Library patron call 602-255-5578 or 1-800-255-5578 to request a library card number.
• Did you forget your library card number? We can email it to you.
