  User Name = Please enter your last name
  Number = Please enter your library card number which was supplied to you by the State Library

• If you do not have a library card and are an Arizona state employee:
  User Name = Please enter STATE GOVERNMENT
  Number = Please enter your 10 digit work phone number, starting with the area code.
  
• To apply for a library card, as a state employee, please complete the online application for a library card.
